Referrals

All program requests or individual referrals for licensed child care programs will go through the Special Needs Access Point (SNAP).

We have created an easy to use “Decision Tree”, which is a simple decision-making tool to help us determine what support or service may be needed, based on how you answer the questions provided.  Once we determine what supports are appropriate, you may be asked to fill out an online form as well as sign consents, as needed.
